Earn My Health Pays rewards when you complete healthy activities such as annual screenings, tests and other ways to protect your health.
Use Your My Health Pays rewards to help pay for:
- Utilities
- Transportation
- Telecommunications
- Childcare services
- Education
- Rent
Or, you can use them to:
- Shop at Walmart for everyday items**
**Rewards may not be used to buy alcohol, tobacco, or firearms products.
2025 Healthy Activities Eligible for Rewards
- $10 Member Portal Registration-Visit OklahomaCompleteHealth.com and click “Login”.
- $10 Health Risk Screening. Once per enrollment.
- $10 Child Well Visit. Ages 3-20. Once per year.
- $10 Adult Well Visit. Ages 21+. Once per year.
- $25 Infant Annual Well Visit. Ages 0-15 months. Earn one reward for each visit. Child can receive up to six well child visits prior to reaching 15 months old.
- $25 Infant Well Visit (15-30 months). Ages 15-30 months. Earn one reward for each visit. Child can receive up to two visits prior to reaching 30 months old.
- $5 Flu Shot. Once per calendar year.
- $10 Breast Cancer Screening. Ages 50-74. Once every two years.
- $10 Cervical Cancer Screening. Ages 21-64. Once every three years.
- $25 Diabetes HbA1c Test. Ages 18-75. Must have a current diagnosis of diabetes to qualify. Once per calendar year.
- $30 ReadyforMyRecovery. Requires six month of continuous engagement in Substance Abuse Disorder services. Earn up to two rewards per calendar year.
- $50 Tobacco Cessation Program. Once per calendar year.
- $25 First Fill of Tobacco Cessation Medication.
- $25 Immunization for Adolescents (IMA). Ages 10-13. Requires one dose Mengingococcal, one dose Tdap, and 2 doses HPV. One reward per lifetime.
- $25 Childhood Immunization Status. Ages 0-2. Requires completion of dose series. One reward per lifetime.
For Pregnant and New Moms
- $25 Notification of Pregnancy Form. Must be completed within first trimester.
OR
- $10 Notification of Pregnancy Form. Must be completed within second trimester.
- $25 Prenatal Visit*. Once per pregnancy. Prenatal date of service must be prior to delivery, postpartum visit, miscarriage date of service, or due date + four weeks, which ever comes first.
- $10 Postpartum Visit *. Once per pregnancy and within 7-84 days of delivery.
* To be eligible for this reward, you must notify us you are pregnant prior to having your baby by submitting a completed Notification of Pregnancy (NOP) form.
Your My Health Pays reward dollars are added to your rewards card after we process the claim for each activity you complete. If you are earning your first reward, your My Health Pays® Visa® Prepaid Card will be mailed to you.
This My Health Pays® Visa® Prepaid Card card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC. Card cannot be used everywhere Visa debit cards are accepted.
Log in to your member account to check your current My Health Pays rewards balance.
Once you complete a qualifying healthy activity, we are notified, and your card will be mailed to you. Remember to keep your card, as future reward dollars will be added to it for each qualifying healthy activity you complete. If you have questions, please contact Member Services.
Once you receive your My Health Pays rewards card in the mail, please follow the instructions on the activation sticker. You will need to have your card with you to complete activation.
To check your balance, login to your Secure Portal account.
Funds expire 90 days after termination of coverage or 365 days after date reward was earned, whichever comes first.
