Don’t risk losing your SoonerSelect benefits

 

Did you know you have to apply for SoonerSelect when you turn 18?

You must apply for Oklahoma SoonerSelect benefits once you turn 18. If you don't apply for Medicaid on time to confirm your SoonerSelect eligibility, you will lose your SoonerSelect benefits.

 

 

 

Why is this important?

 

These benefits may include behavioral and mental health support, transportation and housing support or continuing education opportunities. new information

Applying for SoonerSelect:

  • You should receive a letter in the mail stating that you need to apply for Oklahoma SoonerSelect.  Open all mail from the State and take action right away.
  •  Gather your documents.  Some examples of what you may need:        
    •   Social Security Card
    •   Paystub
    •   Most recent W-2 form
    •   Utility bill
  • Fill out the form completely. Take your time and answer all the questions.
  • Submit the form, including any required documents.
